“Accidents” or acts of aberrant conduct that result in harm to human beings or groups of them in society that occur repeatedly, across time, different individuals, geographic contexts, and situations, and in similar institutions are neither “accidents” nor mere acts of aberrant conduct, but instead part of a social pattern embedded in the standard operating procedures of those institutions, constituting parts of a “social” problem.
